Picture of actual random inspection audit sheet

A good friend is restoring his 69 SS396 Nova and they found this under the carpet. Its dated Dec. 7, 1968 and was wondering if anyone had found this in their camaro. Per JohnZ it is a random audit inspection sheet which I would think would be not that common to find. Anybody else find one? Can you post a picture or supply and information?

While replacing the carpet on my first Camaro, ('67 RS Vert) I discovered a strange lump showing through the back seat passinger side floor insulation that turned out to be a window crank.
'67 RS 327 convertible, '69 RS 350 coupe, '73 Type LT 350 coupe split bumper.

I found 12 new pinch weld clips that hold the mouldings on at the rear of the convertible top well. They were in the slots at the front seat reinforcements, next to the sub frame mounting nut - they had to make noise on hard turns.

This may not apply to 1969, but in 1975 I was a senior engineer at Chevrolet research and development.  I took a bunch of engineers to do drive audits at the Leeds plant that Fall.  It was SOP to do central office drive audits when major changeovers occured.  We drove cars off of the assy line and took them around a pre arranged circuit.  Some may remember that KC had a big flash flood that year.  My company car actually floated away when Rush Creek overflowed it's banks while we were out to dinner!  We lost a lot of cars in the OK car lot at the plant that year too.

It's an audit sheet for Fisher. I've seen similar, but not that exact form.
