Had the same problem after the car sit for a while.  Installed these and "knock on wood" haven't had the problem since.  Comes one to a package.
I also run non-ethanol fuel now.

It's a D80 car.

After almost 40 years, not worth the hassle.  Just buy a new assembly.  Changed the clutch in mine a few years ago working on jack stands.  Almost called a friend to help stab the transmission but after a few tries I got it.

I believe the metal tag was for second gens with limited slip. 
I had one on my 1975 Trans Am.

Always used a bushing, tap it in and done. Bearings seem to have issues.  Never aligned a bellhousing on a clutch install.  Guess the tool people are happy and making money.

Don't know what you have invested in the car.  The right people can fix it just depends on how much you want to put into it.   Did you notice it has a 10 bolt rear end?
