Shipping invoice

I found a factory to dealer shipping invoice for my 68 convertible.  The invoice is in pretty bad shape but near the bottom it says "formerly in company service" along with a name below it.  Anyone know what that means? Does anyone have a decent copy of one of these invoices they could scan that I could look at?

It means a Chevrolet employee used the car for a short period. Could have been at the assembly plant, Central Office, Zone.

It is not this document.  The one I have is smaller and has much less detail.  I will try to scan it again today.  I think I sent you a scan of this several years ago.
