Need to replace the heater core in a 68. Inner fender is already out. Question is, can the removal and replacement of a new heater core be done with out accessing the the heater box from the inside?

Any suggestions or is it pretty straight forward? Replaced one on a big block Nova from the engine side awhile back and it was simple. Was not sure about a small block.

Heater core should  be pulled from inside.  You may also consider replacing the foam facing on the doors.
