And range of accepted date code?

CBTN says 3782608.  Anywhere from a week to a month or two prior to the engine assembly date stamped on the pad is typical.

The 1967 Chevrolet Chassis Overhaul Manual list 1100750 with the following application note, All A/C except w/ RPO M-40 and Chevy II.  I don't know if this is accurate.  Hopefully, those with original cars will reply.

Did 67's have a grommet around the headlight dimmer switch?

Yes, P/N 4591473 installed by Fisher Body.

I couldn't make a straight line with a ruler, and I am sure I could never cut the slots even close to correct or nice looking.  Any ideas who I could look to have this made?  Not even sure who I would contact.

Got one that says "GM Overseas Distribution" Possibly the Netherlands. Bummer... Ya think this might have been a serviceman working overseas and ordered it up? The original Delivery Dealer = Dealer Code 1 Zone 21 . Anybody know what or where that is?...Danny
