Hi,
We're having an issue with a console temperature gauge... the gauge is not working when the ignition is on or when the engine is running.
But when the ignition is off and the headlights are turned on... the temperature gauge pins to the H side of the gauge.
Any advice to what is the issue? possible bad ground?

When the temp gauge pegs to Hot, that says the sending unit side of the gauge has low (or no) resistance to ground; however, the gauge shouldn't do anything without an ignition-switched feed. Yours is apparently getting a 12V feed somehow from the headlight/dash light circuit, and nothing from its ignition feed.

Probably back feeding the temperature gauge circuit off the panel lamps in the gauge cluster. Do the lamps in the part of the cluster with the temperature gauge light up when you turn on the dash lights? If not then the metal plate that holds the temp gauge does not have a ground and it is somehow back feeding thru your gauge and sender to the engine block. What happens if you disconnect the sender with the dash lights on, does it stay pegged, or does it come back down to cold?
